Rates and Insurance
Kos Counseling, LLC is in network with the following insurance companies: Medica, Optum, United Behavioral Health, United Healthcare, UCare, BCBS, and Health Partners. All services are provided to residents of the state of Minnesota, per therapy licensure guidelines. There are a few different ways that services can be covered: through your health insurance (provided that Kos Counseling, LLC is credentialed with your insurance), through your HSA account, by paying the full fee with cash out-of-pocket. You decide which option works best for your situation. See additional information below.
Out-of-Pocket Costs are listed below:
Individual Therapy:
Intake/ initial diagnostic assessment session: $250
Ongoing individual sessions, 50 minutes in length: $175
90-minute sessions: $250
Relational Therapy: (I am not taking couples/ families at this time)
Intake/ initial diagnostic assessment session: $275
Ongoing relational sessions, 50 minutes in length: $225
90-minute sessions: $275
Payment:
Payment is due at the time of service. Check and major credit cards are accepted. Kos Counseling, LLC accepts most HSA and FSA cards. It is your responsibility to check with your employer or healthcare plan administrator in order to determine whether you can use your account for mental health services.
Some insurance companies will allow you to submit a receipt for out-of-network costs for reimbursement. Call the number on the back of your insurance card to find out if your insurer participates in this type of reimbursement.
Cancellation Policy:
Please contact us as soon as possible if you will be unable to attend your appointment. If you cancel in less than 24 hours of your scheduled appointment, you will be responsible for the full fee of your appointment.
